VA's Federal EHR Now Live at Four Ohio and Kentucky Medical Centers

In early June 2026, the Department of Veterans Affairs extended its federal electronic health record system to four additional medical facilities. The deployment covers the Cincinnati, Chillicothe, and Dayton VA medical centers in Ohio, and the Cincinnati VA Medical Center–Fort Thomas in Kentucky, affecting more than 107,000 veterans and about 7,200 clinicians and staff.

The go-live is part of a nationwide modernization effort that resumed in April 2026 with four sites in Michigan. The program was paused in April 2023 following reports of technical issues, system outages, and patient safety concerns that drew congressional scrutiny. During the pause, the department worked to address system problems and standardize its implementation procedures.

The stated goal of the EHR initiative is to create a single, lifelong medical record that follows service members from the Department of Defense through their care at the VA. The system is designed to exchange data with the DoD and private-sector healthcare partners to improve care continuity and reduce duplicate testing.

According to reports, the VA plans additional EHR deployments later in 2026 at facilities in Indiana, Ohio, and Alaska as it continues the rollout across its national network.
